A 32 year old male came to us with an airgun pellet in the L frontal sinus.
Surgery: Frontal trephening,& endoscope guided removal.( see the images,& video)
The pellet, sized 8 mm X 5 mm, had entered the sinus just above the left orbit, & fractured the posterior FS wall,& intending the brain.The three tertiary hospitals consulted before suggested an osteoplastic flap approach, through a coronal incision.

Iatrogenic Bilateral CSF leak repair
Surgery: Bilateral CSF leak,& brain hernia repair.
A 19 year old girl, an Engineering student admitted with meningitis. On examination she had bilateral CSF leak. A CT & MRI showed a bony defect of 12 mm in Right fovea with brain herniation, & a 6mm defect on the left side.
The defects were repaired endoscopically.Since she had a CSOM in the left ear from childhood, I did a Cortical Mastoidectomy with tympanoplasty one year later. But four years later she developed she developed right sided CSF leak again.There was a small defect on nasal endoscopy which was again repaired endoscopically.She is fine on follow up after two years.
